The Mysterious Island is a side quest during Balduran's Final Voyage, one of the quest arches in the Tales of the Sword Coast expansion. This quest goes hand in hand with Balduran's Final Voyage.
Acquisition[]
As you progress through the Isle of Balduran, your journal will update as you discover things about the isle and it's secret. It is initiated by speaking to Solianna, who approaches you as soon as the cutscene is done:
The Mysterious IslandThere must be a settlement on this island, because I met a child far too young to have gotten here on her own.

Talking to anyone on the island will hint at your next step:
The Mysterious IslandA woman by the name of Kaishas can smooth my way with the local villagers.
And a second:
The villagers here all suggest that I talk to their headwoman[sic] before wandering about.
But ultimately the party should indeed talk to Kaishas Gan.
You can refuse her plea:
The Mysterious IslandI spoke with the current leader of the village, Kaishas Gan, and she asked me to help rid the island of "the beasts." I refused, though I may have to reconsider if I want any help leaving this place.
This implies that we may need her help, but it does seem optional. If you do agree to help her, you'll get this entry:
I have agreed to help Kaishas Gan rid this isle of the "beasts," a group of monsters that have apparently been harassing her village for years. They live beyond the village wall, and she said their leader's name is Karoug. He is more wolf than man, I think she said.
There are many side quests outlined in the walkthrough Balduran's Final Voyage. This quest has one task and it is to get rid of Karoug. Your journal will also detail some background information about the island's situation and its inhabitants to aid you in your decision making.

This part of the guide assumes a generally positive attitude and not being in a hurry.
Lahl[]
On the way out of the village, at the end of the farm fields stands the farmer Lahl, concerned with his crop.
An old farmer named Lahl asked that I kill some of the beasts outside the village to avenge his wife. It is the least I can do. Perhaps I can also arrange for these people to be able to venture out beyond their village walls once again.
Once Karoug is dead, you can talk to him again for no reward, but an update in the journal:
Lahl was grateful for my defeat of Karoug. I have made a lot of friends here. He said I would make a good addition to the village.
Jorin[]
Outside the village there's a man named Jorin (2437.1944
), who's not very impressed by our claims:
The gruff Jorin didn't seem too impressed with me. I suppose that's to be expected from one of the village protectors, who must be suspicious of any outsider.
Taloun[]
A sailor can be met in one of the storage houses (849.2453
) and carefully navigating the dialogue will reveal more about the island's and Balduran's history.
We met a former sailor from Amn in the island village. He explained to us that Balduran's crew was attacked by savage wolfweres and holed up here where the village now stands. He believes that Balduran and his guide Dradeel died at the ship. He went on to say the husband of the headwoman attempted to make it to the mainland in order to effect the rescue of his clan and has not returned.
Additionally, we keep in mind that there may be more than meets the eye:
Taloun has warned us to trust no one on the island. He seemed to be serious.
Meym[]
Meym can be found on the North island, close to the Siren Queen. He seems to be there just for story and a fight. Talking a second time to Meym makes him attack the party and adds a new entry to your journal
I have met Meym, one of the "beasts" that have plagued this isle. He gave the impression that Kaishas and her people were not so different from he and his. We shall see.
Durlyle / Delainy[]

As this character is a historian it's a safe bet they know a lot of what has transpired here and on the ship. When the party returns the cloak and shows interest in the history (and historian), additional background information is made available:
The Mysterious IslandThe villagers came to this island from somewhere in the far west, brought here by a group they call "the explorers." When it appeared they might be thrown overboard, they fought back. Those who survived made their homes here.
The villagers here say of themselves that "they belong." Apparently outsiders do not, at least not until they perform some sort of ritual.
One of the "explorers" was named Balduran. Rather than allow the ship to continue, he insisted on scuttling the vessel.
No one in this village can tell me precisely what happened on the ship that brought them here. All they know is that they were once bound but now they are free.
