This "Skull" is a plot items obtainable only during the Baldur's Gate II: Throne of Bhaal portion of the game. It is a "critical item" and can't be sold.

Found in a container (a chest) within the Saradush Prison, in the eastern side of the complex.
Gameplay[]
A necessary item needed to resolve The ghostly apparition questline.
With this skull in the party's possession, then when exploring further in the prison - inside a cell - will trigger a text display over a 'container' (shallow grave):
- In the corner of the room is a shallow grave which has been defiled. Within is a human skeleton, though the skull is missing.
Place this skull and the Rusted Amulet in the unmarked grave, then sanctify the grave using a dose of Elven Holy Water - see the quest article for what transpires and other details.
