Marek's Potion of Antidote can be obtained only from Marek himself in Chapter Five. It is the end goal of the quest Marek and Lothander and is needed to cure the party of a deadly poison in Baldur's Gate: Enhanced Edition. It can be used up to ten times but only the protagonist needs to drink it, and only once.
In the Original BG1, this item is not actually the cure to Marek's poison. To be healed, the player must talk to Marek towards the end of the quest, or talk with Tethtoril in Candlekeep before the poison kills them. Only the Marek route will provide XP and an acknowledgement that the party was healed.
