Kuo-toan Priest's derive their clerical abilities from the intensely insane fervor they attain when they worship the deity Blibdoolpoolp. They conducted sacrifices in Blibdoolpoolp's name using ceremonial drowning pools to drown captive humanoids or kuo-toa who exhibited indignant or rebellious attitudes. They were responsible for running kuo-toa civilizations and kept black pearls and large crustaceans in their temples which they used to ascertain favorable omens from their patron goddess.

Two types of kuo-toan priests are encountered in the Baldur's Gate series: the KUOPRI01.cre and the GORKUO03.cre with the only differences between the two being that the GORKUO03.cre is a level 12/12 cleric/thief with 82 hit points and 4000 XP.
Locations[]
The KUOPRI01 creature is encountered in the Underdark, and the Western Tunnels. A kuo-toan priest is also kept as a prisoner inside the Mind Flayers in Underdark area.
The GORKUO03 creature is encountered in the cells of Abazigal's Lair along with other kuo-toan prisoners.
Gameplay[]
When an enemy is detected, the Cleric / Thief class priest will force cast Defensive Harmony and Strength of One buffs, which may also affect any nearby allies.
As denoted in the InfoBox the priest is scripted to attempt to cast a number of disabling and offensive spells at enemies, as long as they are still available in memory. A few can be attempted more than once.
Various healing spells will be attempted on itself and even nearby allies if damage level thresholds have been met.
They fire kuo-toan bolt ammunition from their Light Crossbow when not spell casting.
If forced into melee, they wield a specific unenchanted spear that deals 1d6+4 piercing damage – including strength and weapon proficiency bonus – and will slow and grease effect the target for 15 seconds unless a save vs. spell is made.

Kuo-toan Priest wear an undroppable Ring that provides some additional powers and protections:
- Invisible Detection by script
- Immune to Paralyze and Held effects
- Immune to Webbed effects
- Immune to Grease effects
- Immune to Slow effects
- Immune to 10 specific spells related to above
- Magical damage energy resistance 90%
- Physical damage resistance 33%

Installation of the Sword Coast Stratagems Mod provides the creature with SCS AI scripts for better tactical decision making, defense and targeting behavior.
Spell usage and self-buffing is more aggressive with the "Smarter Priests" component installed.
Additionally, the "Legacy Weapon Proficiencies" are removed, and instead 1 pip in crossbow and spear weapon proficiency are provided to align with the Cleric/Thief class.
Depending on the game difficulty setting, SCS will add at least one extra priest to groups of Kuo-toa at the Underdark Bridge, and in the Western Tunnels.
