A kuo-toan captain usually leads groups of kuo-toa in Baldur's Gate II, consisting of warriors, whips, priests or wizards.

- Watcher's Keep Elemental Level – a kuo-toan captain may appear in the air library as part of a area script spawn trigger, depending on the party's XP levels
- Watcher's Keep Final Seal Dungeon – two of them, together with a kuo-toan priest and a kuo-toan wizard, when operating the Globe Machine
Gameplay[]
- Kuo-toan captains have True Sight, besides their general bonus to invisibility detection.
- They fire Kuo-toan bolts that have a chance to stun.
- If forced into melee, they use a specific, magical Spear, though without being enchanted, that deals 1d6+7 piercing damage – including strength bonus – and will slow the target for 15 seconds if no save vs. spell is made.

- Kuo-toan captains receive their resistances by an item, rather than being set in their creature code, as usual. The Magical damage resistance is set to 90, while the physical resistances work as bonuses.
