Fear (Fiend) is present when the party descends stairs leading below to the Fear (Abyss) area during Baldur's Gate II Chapter 7, part 2.

A Tear of Bhaal lies very close to here, child of Bhaal... just beyond either of these two rooms before you.

Gameplay[]

The Fear Fiend will initiate and administer the Test of Fear. The player's bhaalspawn PC will be presented dialogue choices to navigate the test, which will determine if the final choices made will be determined as either the "good" or "evil" path.

The player's goal in all of the various tests is to obtain one of the Tears of Bhaal - which are required to complete the Finding Irenicus in Hell questline, and eventually lead to unsealing The Abyssal Door (also referred to as the "Watcher's Door").

Those dialogue choices during the test determine what reward is assigned to the protagonist when a Tear of Bhaal is added to the door. It may also result in an alignment shift.

Unlike some of the other demons presenting the Tear of Bhaal tests, this particular one can be attacked by the party if desired. If this occurs a Journal entry will display:

Finding Irenicus in Hell:
I encountered a demon that seemed to perform some kind of test of character. It explained that there are two paths to a Tear of Bhaal... and both are paths of fear. It offered me a cloak of nymph's skin to protect me from the fear, saying the path would be easier. I attacked the demon and it fled, taking the cloak with it. I suppose I shall have to gain the Tear with my own power.

The demon will leave the map when this happens and can't be killed by the party (it is equipped with the MINHP1.ITM protective item). The fiend is simply a plot actor to present the specific Tears of Bhaal test involving fear.
