When a demilich casts the innate spell Demilich Howl, a literal shriek like a banshee is cried out by the undead caster.

At the culmination of this dreadful spell, the demilich shrieks like a banshee. Everyone in the area of effect hears the awful cry. Those who fail a Saving Throw vs. Death die instantly. The caster and allied forces are immune to the effects of the spell.

Upon the culmination of the long casting time (almost a full round), the spell exposes all non-allied targets in the area of effect to a Slay effect. Each creature (not allied with the caster) must make a Saving throw against Death or perish. The spell has a unique audio file assigned to it.

As this is an AoE spell, spell protections such as Spell Deflection or Spell Trap don't provide any defense.

This spell can be blocked by:

If an exposed creature missed their saving throw and isn't able to block the spell with magic resistance or be immune to death effects, then a lighting effect plays over the victim, and a ghostly figure animation rises from the now dead creature. The corpse is still intact and can be revived with a Resurrection effect.

The Sword Coast Stratagems mod has changed this innate spell, revising its saving throw to include a -4 penalty, and the howl is no longer affected by magic resistance.
